Job Description:
The Continuous Quality Improvement Registered Nurse (CQI RN) is
responsible for coordinating and managing the Ryan White Program's
(RWP) clinical quality management (CQM) activities and data
reporting. The responsibilities of this position include assisting
in the coordination of HIV+ patient's activities and services, such
as completing referrals for new and return to care patients and
transportation, reviewing and updating RWP services, work plans and
CQM activities. In addition, the CQI RN will collects, enters and
reports all required data in a timely manner for the various grant
reporting requirements. The CQI RN person works with other agencies
to enhance a system of care and may serve as a community
spokesperson for the Ryan White Programs.Job
Responsibilities:Demonstrates the ability to coordinate and manage
the Ryan White Program's (RWP) clinical quality management's (CQM)
activities and data reporting.Demonstrates the ability to assist in
coordinating HIV+ patient's activities and services, such as
completing referrals for new and return to care patients and
transportation.Understands the goals, objectives, clinical
guidelines and quality management of the RWP.Works closely with the
Director of Social Services in reviewing and updating RWP services,
work plans and CQM activities.Accurately collects, enters and
reports all required data in a timely manner for the various grant
reporting requirements.Effectively manages the CAREWare system and
other databases by ensuring the integrity of the data through data
reportingMaintains effective data collection and evaluation reports
in accordance with federal requirements.Completes and reconciles
the daily RW charges in Allscripts PM, EHR, and CAREWare to compile
into the monthly RWP report.Reports at least monthly to the
Director of Social Services regarding the CQM activities of the
Ryan White programs and explains, clarifies, or interprets issues
which may impact data reporting.Attends meetings and/or trainings
of the Ryan White Program, the SeAETC trainings, and other
work-related meetings and trainings.Actively participates in
meetings with the Ryan White Program staff and the social services
staff to ensure staff coordination.Runs, analyzes, and reports RWP
performance measures accurately on a monthly basis and/or when
applicable.Accurately completes the Ryan White Work Plan and
reconciles it with monthly RW service charges and performance
measures data.Completes and documents Rapid HIV testing per policy
and procedures and accurately completes SeMRHI and MSDH Rapid HIV
testing documents.Generates an encounter for all face-to-face case
management visits and/or Rapid HIV tests on the date of
service.Documents face-to-face, telephone, Rapid HIV testing, and
other patient-related encounters in the patient's electronic health
record and/or CAREWare within 24 hours of the date of
service.Demonstrates cultural competence in all activities and
ensures that all patients are fairly treated with high quality
careDemonstrates a clear and ongoing understanding of patients'
confidentiality and privacy standards.Maintains prompt and regular
attendance.Verify timecard at the end of each pay period.Performs
related work duties, as assigned by supervisor.Job
Requirements:Must be a Registered Nurse with a current license in
the State of MississippiMinimum of three years nursing
experienceKnowledge of social services and health care delivery and
data systemsMust possess a RN licensure and current Basic Life
Support (BLS) through the American Heart AssociationMust
continuously use sound communication and counseling skills in
coordinating RWP data activitiesMust be able to deal effectively
with patients and staffPhysical and Other Requirements:Must
continuously listen, visualize, and have dexterity and eye-hand
coordinationAbility for simple grasping and speed workMust
frequently sit, squat, reach, use both right and left forearm
rotation, walk and climb stairsMust occasionally kneel, twist, have
ability to grasp firmly, lift and carry, push and pull in excess of
10 lbs.
