Job Description:
----- - Provides skilled nursing care and treatment to clients
in their homes following physician's orders and care plans.
Accurately documents care, health status, and communicates updates
to families and physicians.
Administers medications, conducts assessments, and offers health
education to promote client independence.
Completes necessary documentation, including start of care and
supervisory visits, ensuring compliance with regulations.
Collaborates with the nursing team to evaluate and plan care,
attends meetings, and maintains confidentiality.
Ensures medication management, safety measures, and proper use of
equipment and supplies.
Communicates effectively with team members regarding client status
and hospitalizations.
Provides training and may cover shifts on weekends and holidays as
needed.
Qualifications include a nursing degree, current license, CPR
certification, and preferably home health experience.
