Care Transition Coordinator, you will: Cultivate and nurture
relationships with referral sources, patients and families Be a
source of knowledge and information about our service offerings,
the unique elements of hospice and the benefit of hospice services
for patients and families facing the end of life Utilize available
tools and market knowledge to understand market dynamics and
identify productive sources of growth Collaborate with operational
leaders to support a strong team culture, address challenges,
promote accountability and drive continuous improvement
Consistently deliver on monthly individual performance goals Skills
for Success Committed to delivering outstanding customer service in
every interaction Solution-driven, execution-oriented, and
competitive Able to overcome obstacles and challenges and always

timelines Experience to Deliver on our Mission College degree,
valid state driver's license, and reliable transportation. One year
of hospice or healthcare sales experience. Knowledge of business
and fiscal management, governmental regulations, and accreditation
