Responsible for covering all inpatient units throughout the
hospital, focusing on social work case management for a variety of
complex patients. Will directly report to the Social Work Manager.
Responsibilities: Manage a case load of 15-20 patients per day.
Perform medical and social discharges as needed. Collaborate with
RT and Pharmacy for discharge planning. Work alongside a team of 15
full-time and 17 part-time Social Workers. Requirements: MUST HAVE
active MS License and EPIC charting experience. Minimum of 2 years
of Acute Care Management experience required. BLS Certification
required. Previous experience in a large academic medical center
hospital/Level 1 trauma center is required.
