Under the direction of the Director of Social Services, the Ryan White MAI Outreach Worker/Educator is responsible for developing and implementing a program of outreach and HIV Counseling, Testing and Referral (HIV CTR) to the targeted high risk population for HIV infection based on surveillance data; for providing education, counseling and testing to the community regarding HIV education and prevention; and for compiling accurate monthly data reports on the outreach activities and HIV testing. This staff person will also work with the Ryan White staff to ensure linkage to care for newly diagnosed HIV positive patients. The Ryan White MAI Outreach Worker/Educator is a community spokesperson for the Ryan White programs, and works with other agencies to enhance a system of care. Demonstrates knowledge and skills to appropriately communicate and interact with the staff, patients, families and visitors of all age groups while being sensitive to their cultural and religious beliefs. SeMRHI requires all employees to show proof of vaccination for both COVID 19 and annual influenza or provide a religious or medical exemption prior to the beginning of employment.Job ResponsibilitiesDemonstrates the ability to effectively coordinate outreach activities and services, especially to the targeted high - risk population.Understands the goals and objectives of the Corporation as set out in the By-Laws and Policies and Procedures Manuals. Understands the goals and objectives of the Ryan White Program including the Minority AIDS InitiativeMaintains and reports accurate data for outreach activities regarding HIV testing (Rapid and Serum), events, and presentations to the Director of Social Services by the tenth of each month.Works closely with the Director of Social Services in reviewing and updating program services and outreach activities that are contingent upon MSDH HIV surveillance data.Effectively counsels, educates and responds to the needs of the community about HIV education, prevention and testing.Effectively monitors all of SeMRHI's Rapid HIV testing sites to ensure compliance of the testing protocol, completion of all testing and condom logs, maintain adequate supplies of condoms and test kits, and accurate reporting of testing data monthly.Participates in meetings of community groups, local organizations, professional groups and regulatory agencies to promote the outreach activities of the Ryan White program.Establishes linkages with public and private schools, colleges and universities and community agencies and/or organizations to provide HIV education, prevention, and testing events.Participates actively in Performance Improvement activities and monitors problems discovered.Works closely with the medical case managers to ensure that newly-diagnosed HIV positive patients are linked and enrolled into HIV care within 90 days of diagnosis date at SeMRHI.Actively participates in meetings with the Ryan White and social services staff to ensure care coordination.Attends health fairs targeted to the high-risk patient population hosted by various community organizations to advocate for the Ryan White Program's services offered at SeMRHI and to promote HIV education, prevention and testing.Administer pre-test and post-test counseling, including safer sex practices, PEP and PrEP, for those who receive a Rapid HIV test and provide an accurate test result to the patient.Generates an encounter for all Rapid HIV tests conducted on the date of service.Documents Rapid HIV testing and other patient-related encounters in the patient's electronic health record within 24 hours of the date of service.Reconciles accurately the number of HIV tests (Rapid and Serum) conducted at each testing site on the Daily Activity Report (DAR) by the tenth of each month.Demonstrates positive personal attributes such as honesty, integrity, and professionalism in all aspects of the job.Demonstrates cultural competence in all activities and ensures that all patients are fairly treated with high quality care.Demonstrates a clear and ongoing understanding of patient confidentiality and privacy standards.Maintains prompt and regular attendance.Verifies timecard at the end of each pay period.Performs related work, as assigned.Job Requirements:B.S. degree from an accredited college or university in Health Education, Public Health, Social Work, Nursing or a related fieldAt least 3 years of experience in a community health or health education settingValid license as a Social Worker or Nurse in Mississippi as requiredValid Mississippi driver's license a mustKnowledge of social services and health care delivery systemsExperience in group education and tracking patients usefulAbility to continuously use sound communication and counseling skills in handling Ryan White case management activitiesAbility to reason in planning and reportingGood oral and written communication skillsAbility to deal effectively with patients and staffPhysical and Other Requirements:Continuously listen, visualize and have dexterity and eye-hand coordinationFull-range body motion which includes frequently sitting, ability to reach, use both right and left forearm rotation, walk, stand, kneel and twistAbility to grasp firmly, lift and carry, push and pull in excess of 10 lbs.
